Transcript Request Form
This form must be completed
at least THREE WEEKS before your first college application/scholarship deadline. It should be emailed back to your counselor.
Release of Records
This "one time" pink form is signed by both student and parent and placed in the Guidance File. It authorizes Guidance to release transcript information to colleges/coaches.
Brag Sheets: Students must complete Brag Sheets on Naviance (About Me Tab > Surveys).
- The Student Brag Sheet provides additional information for the counselor to incorporate into your recommendation letter and should be completed by the start of a student's senior year.
- Teacher Brag Sheets must be completed at least 3 weeks before your first college deadline. Please notify your teacher(s) when you have completed them.
- Parent Brag Sheet: This form can be downloaded and completed or can be filled out on the Parent Naviance account (About Me Tab > Surveys). This should be completed by the start of a student's senior year.
1st Quarter Grade Request
Complete this form if you would like your 1st quarter grades sent to your colleges. MLHS does not send these grades out automatically since not all colleges require them. That said, students who are proud of their 1st quarter grades are encouraged to send them anyway.

Graduate Transcript Request
Students that have graduated Mountain Lakes High School should complete this form should they need their transcripts sent to an outside organization. All completed forms should be sent to Mrs. Gregory at [email protected].
OTHER FORMS
Student Opt-out Form
Under the federal "No Child Left Behind" Act, public high schools must give the names, addresses and telephone numbers of students to military recruiters, college/university recruiters and prospective employers if the recruiters request the information (P.L. 107-110, Section 9528; 10 USC 503). However, students or their parents have the right to instruct the school in writing that this information is not to be released. This is the opt-out document.
